Workplace Injuries

***If your injury is life- or limb- threatening dial 9-1-1***

If you sustain a non-life- or non-limb- threatening injury while on the job, please take the following steps:

1. Notify your supervisor immediately.
   
   1. Fill out an Incident Report Form (see link below) and turn in to Human Resources within 24 hours.
2. Either you or your supervisor should call the Company Nurse Injury Hotline at 1-877-518-6702.

3. Seek the medical attention necessary for your injury.
4. Human Resources will follow-up with you regarding the details of your injury.
